Clinical assessment shouldn't stop at the consultation room.

Outpatient clinics and rehabilitation centres see patients in episodes. The clinical reality is continuous. We close the gap between visits with longitudinal patient data your clinicians can actually use — captured from the patient's environment, integrated with the clinical pathway you already run.

Episodic care, continuous condition.

Rehabilitation, chronic disease management, and outpatient follow-up are all longitudinal by nature. Clinical observation, by contrast, is concentrated in short consultation windows. The result is a structural information asymmetry: clinicians make decisions on a fraction of the relevant data. Generic remote monitoring tools narrow the gap. They rarely close it, because they are built around vital signs alone — not around the clinical pathway.

Designed around the pathway, not the device.

The platform captures longitudinal patient data — physiological, behavioural, and pathway-specific functional measures — between clinic visits. It is configured against the rehabilitation or outpatient pathway your clinic actually runs, presents the relevant signal in the clinical workflow your team already uses, and produces a structured longitudinal record that supports clinical decision-making across the episode of care.

What the platform delivers in a clinical context.

Capability what it means in practice

Between-visit longitudinal data

Continuous physiological, functional, and behavioural signal capture aligned with the patient's care pathway.

Clinical workflow integration

Data surfaces in the EHR or clinical workstation your clinicians already use.

Personalised baselines

Patient-specific thresholds, not population averages.

Pathway-specific configuration

Rehabilitation, chronic disease, and outpatient follow-up pathways configured individually — not a generic dashboard.

Functional outcome tracking

Structured measurement of pathway-relevant functional outcomes between assessments.

Decision support, not decision replacement

Clinical recommendations remain with the clinician. The platform delivers the signal, not the verdict.

Research-grade data architecture

When partnered with an academic group, the same deployment supports clinical service delivery and research-grade data capture in parallel.

Built to live inside your clinical workflow.

The platform integrates with the EHR, clinical documentation, and assessment tools your clinicians use today. Clinicians do not need a second login, a second dashboard, or a second source of truth.

What a clinical pilot looks like.

1. Discovery (4–6 weeks)

We map the clinical pathway in scope — rehabilitation programme, chronic disease cohort, or outpatient follow-up service — together with your clinical leads, IT, and operational leadership. We define the pilot cohort and agree on outcome measures.

2. Deployment (8–12 weeks)

We configure the platform against the pathway, integrate with your clinical systems, equip the participating patients, and onboard the clinical team. Training is delivered to clinical, allied health, and support roles in parallel.

3. Operation & evaluation (3–9 months)

The platform runs in production conditions across the pilot cohort. We meet monthly to review clinical outcomes, surface workflow issues, and refine configuration. The pilot closes with a written clinical evaluation and a defined path to expanded deployment.

Who this fits.

This fits if you are

An outpatient clinic, rehabilitation centre, or clinical network operating defined care pathways

Running structured, longitudinal patient cohorts (rehab, chronic disease, post-acute)

Accountable to clinical governance and a defined regulatory regime

Already operating an EHR or clinical workstation

Open to a research-grade pilot, alone or with an academic partner

This is not a fit if you are

A single-clinician practice below institutional scale

A direct-to-consumer health app

A retail health subscription service

Looking only for off-the-shelf wearable monitoring

Looking for a packaged product with no clinical configuration

Built against the regulatory contexts you operate in.

The platform's deployment architecture is aligned with EU MDR class IIa medical device pathways where clinical claims apply, GDPR for all EU deployments, and HIPAA-compatible configurations for US deployments. Specific certifications and conformity assessments are scoped per engagement.
